JOB ANNOUNCEMENT

Social Work Case Manager

San Diego Youth Services seeks a Clinical Case Manager in the Resource Family Agency (RFA) to be based at the San Diego office location. This is a full-time & exempt position reporting to the RFA Program Manager.

This position is currently funded through a contract through December 2025. While continuation of the position is not guaranteed, SDYS is committed to making every effort to secure additional funding to maintain the role past the current funding period.

San Diego Youth Services has helped improve the lives of more than 780,000 homeless, runaway and vulnerable youth since its beginning in 1970. We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing to more than 28,000 youth every year at 100+ community and school locations. Through prevention, early intervention and treatment, we also help youth before they need higher levels of care or become homeless.

We aim to:

- Meet the basic needs of youth. Hunger, poverty and youth homelessness are on the rise for many youth and their families today. We help meet these basic needs for youth. What could be more vital than food and shelter? We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing.

- Create positive connection. We help youth focus on positive life changes and healthy relationships as part of providing needed resources and services. San Diego Youth Services has long provided foster homes and adoption support services for families considering adopting a youth in the foster care system.

- Help youth develop life skills. In addition to a range of safe and stable housing options, we offer support to help youth develop the independent living skills necessary to become self-sufficient and keep off the streets.

- Promote overall health and well-being. Early intervention and care for the whole child are critical. We provide a broad continuum of services to help youth overcome trauma and challenges that put them at risk, including case management, counseling, mental health treatment and educational support and resources.

SDYS has a staff of approximately 250 employees and 80 volunteers working across San Diego County and an annual budget of approximately $24 million.

DEPARTMENT DESCRIPTION OR PROGRAM DESCRIPTION HIRING FOR

San Diego Youth Services is one of the region’s best providers and source for foster care opportunities. Utilizing our one-to-one approach, we create outstanding outcomes for foster families. Our foster families provide safe, stable and healing home environments for children who have experienced neglect and abuse. Our services also include a focus on helping current and former foster youth transition into thriving, self-sufficient, self-actualized adults.

CONTEXT OF THE POSITION

Accountable to the Program Manager. Responsible for the placement and supportive care of children/youth in certified resource family agency homes. Responsible for the management of a caseload including development, documentation, implementation, and evaluation of client plans, supervision and reporting of the same.

Essential functions include:

- Participate in the matching, transition, and placement of youth in foster homes

- Responsible for implementation, compliance, and documentation of Community Care Licensing regulations, San Diego County Contract, and SDYS Policies and Procedures which also includes and is not limited to regular participation in the following:- Individual supervision/group supervision

- Treatment Team meetings

- Foster Parent training

- Foster Parent support groups

- Foster Parent meetings

- Child and Family Team meetings

- All applicable agency training and meetings.

- Provide assessment, risk assessment, crisis intervention, Client plan development, progress notes, quarterly reports, incident reports and case management which includes documentation in client EHR in a timely manner.

- Assess foster parent(s) needs related to meeting treatment objectives and provide support and assistance in developing appropriate parenting techniques and emotional climate.

- Advise and support foster parents on compliance of CCL regulations. Monitor the foster family homes to ensure that they are in compliance with governing rules & regulations.

- Maintain professional relationships and contact with placement workers, case managers, therapists, teachers, advocates, attorneys, psychiatrists, doctors, dentists and other child welfare professionals.

- Visit foster homes weekly.

- Respond to service partners during a crisis or urgent matter, including those that occur outside of normal business operating hours, by implementing and following the on-call procedures whenever the rotation of on-call falls into the scope of responsibilities for the assigned period.

- Perform other related duties as assigned.

PREFERRED PROFESSIONAL EXPERIENCE AND QUALIFICATIONS

- Master's degree in social work, marriage and family therapy, or related field, and field experience/practice in public or private social service agency at a master degree level, 18 months experience working with children and families and two years’ experience in public or private child welfare services setting required.

- License-eligible in Marital and Family Therapy, Social Work, or Psychology by the state of California preferred.

- Semester units and/or related coursework can be considered in place of experience. Must provide transcripts. Familiarity with child welfare system, youth who have experienced trauma, and case management experience preferred.

- Must be able to work within and represent a culturally and ethnically diverse community. Bi-lingual Spanish-English preferred. Must complete clearances including child abuse/criminal background check, health and substance abuse screening.

- Must have a reliable vehicle, auto insurance, and good driving record. Must be willing to transport youth.

- Must be willing to develop or possess working knowledge of trauma-informed care and dual diagnosis treatment; experience working in trauma-informed and dual diagnosis programs preferred.

- Must be able to obtain/maintain CPR and First Aid Certification and pass all necessary background checks, including the Community Care Licensing clearance.

PHYSICAL REQUIREMENTS:

- The ability to bend at the waist, kneel, or crouch to assist youth.

- The ability to communicate information and ideas so others will understand, to exchange accurate information and make presentations.

- The ability to lift and carry 20 pounds-lifting books, materials, and equipment to execute lessons and presentations.

- Able to observe details at a close range and interpret the information (such as reading), prepare, and review a variety of documents related to activities of the youth and to be able to monitor youth activities.

- Sitting or standing (stationary position) for extended periods of time.

- Traversing extended lengths to move around campuses.

- Potential exposure to physical injury from aggressive behavior.

Salary Range: $31.00 – 33.00/hour
